- Students date of birth
- Students reference number (if known by University)
- Name of HEI
- Name of course
- Name of Local education Training Board (LETB)
- Cohort (In the form MONTH/YEAR)
- Start date of course
- Last date of attendance
- Nature of withdrawal
- Details of completing HEI officer and date completed
