If a patient lives in Scotland or Wales but are registered at a GP surgery in England, they may be able to apply for an entitlement card to exempt them from prescription charges at pharmacies in Scotland or Wales.

Entitlement cards will be issued free of charge to eligible patient's.

The patient must show the entitlement card at the time the prescription is presented for dispensing.

The address details on the entitlement card must match the address details on the prescription form.

FP10's dispensed at a Welsh pharmacy

If the patient has a Welsh exemption, they must be sorted into Group 1A before submitting for manual checking and payment.

You must indicate the patient charge is '£0.00' and write on the reverse the card has been presented and verified. You must include any reference or NHS number.
