Private code

A private dispensing or prescribing code is needed on the FP34PCD form.

Pharmacy

You should enter your private dispensing code, this is separate to your normal OCS code.

Practice

You will not have a private dispensing code. You should use the private prescribing code of a doctor at the practice, whether it’s the doctor who prescribed the prescription or the senior partner.

Submitting the FP34PCD

The FP34PCD form must be submitted to Prescription Services processing division in Newcastle. This is regardless of where you send your normal monthly submission.

The address is provided on the FP34PCD form.

If you send your monthly submission to Newcastle, do not include your FP34PCD form with this. Both batches should be kept separate.
