Private code
A private dispensing or prescribing code is needed on the FP34PCD form.
Pharmacy
You should enter your private dispensing code, this is separate to your normal OCS code.
Practice
You will not have a private dispensing code. You should use the private prescribing code of a doctor at the practice, whether it’s the doctor who prescribed the prescription or the senior partner.

The FP34PCD form must be submitted to Newcastle Greenfinch.
The address is provided on the FP34PCD form.
Your usual monthly batch and your FP34PCD submission must be kept separate.
