If the FP10 is computer generated and your age or date of birth is clearly visible, a signature is not required.

You do not need to tick the box on the back of the prescription. This is because you’re classed as automatically age exempt.

If your age on the front of the prescription says 59, but you’re collecting it after you've turned 60, you must sign and tick the exemption on the back.

If the prescription is hand written, the FP10 must be signed and the relevant box on the back of the prescription must be ticked.
