Should a patient sign and complete the declaration on the back of the white EPS token?

A patient must complete the declaration on the EPS token.

The only exception to this would be for patients who are age exempt with their date of birth contained within the electronic message.

These declarations can be collected on either: 

• the dispensing token printed by the pharmacy 
• the prescribing token given by the prescriber
