Can a refund be paid if a patient presents an old FP57?

You may be presented with the FP57 / WP57 (0403) version of the form. If a patient does produce a 0403 form, the refund may be paid as long as either the form is presented within three months of the charge being paid, or the patient has an authorised form LIS04(P).
