Which contract should I submit the completion of an open course of Orthodontic treatment on if my contract number has since changed?

In this scenario an orthodontic closure form should be submitted on the new contract, as this is open when the treatment is finished. 

They should only be dated with the date of discharge.

Both the start date and the completion date must be the same on the completion form, that being the discharge or completion date.
