General dental treatment

The completion date for a Failed To Return (FTR) / Incomplete Course of Treatment (CoT) would need to be the date of the patient's last visit and is a mandatory requirement.

The patient would pay the relevant charge for the band of treatment completed however, you would receive the UDA for the treatment that you have actually started.

Example: The patient had a check up and a band 3 CoT was planned for a denture. However, the patient didn't return to have the denture fitted but the practice have taken the impression. The patient would only pay the band 1 charge for their examination. The practice would claim for band 3 and incomplete band 1 on the FP17 claim. The full 12 UDA would be awarded.

Orthodontic treatment

If the orthodontic appliance has been made but the patient has Failed To Return for the fit, you need to submit the claim as 'Assess and refuse'.
 

The two month rule for submitting FP17 claims doesn't apply to a FTR CoT.

If the band started is the same as the band that has been completed so far, you should still tick both the band and the incomplete band on the FP17 claim form.
