Requests from dentists

You can request a patient's review by a Clinical Advisor if you:

  • have a disagreement with a patient about a clinical issue
  • need assistance with a treatment plan
  • have a complaint about the interpretation of the GDS or PDS regulations

You can request a patient to be reviewed by a Clinical Advisor by completing a D7 form.

Requests from commissioners

You can request a patient's review by a Clinical Advisor to:

  • assess the quality of treatment provided
  • assess whether treatment has been appropriately provided
  • assist with the resolution of a complaint

You can request a patient to be reviewed by a Clinical Advisor by completing a D7A form.

How to apply

Contact NHS Dental Services to request a D7 or D7A form.

We're unable to consider a request if:

  • the patient has been seen by a Clinical Advisor within the last 12 months for the same issue
  • the request is for advice about a claim for damages or compensation under civil litigation

Once a patient has been examined by a Clinical Advisor, a report will be produced with their findings within 2 weeks of the examination.
