How do I set up or amend codes for prescribing private controlled drugs?

An authorised signatory at the Integrated Care Board (ICB) must inform us of any: 

• additions
• deletions
• changes

They must notify us by using one of the relevant proformas on our website.

The completed form should be returned to prescriptioninformation@nhsbsa.nhs.uk.

Dentists

A generic Private CD Dentist code will be issued to the ICB. This code is to be used by all dentists private prescribing within their area.
