An authorised signatory at the Integrated Care Board (ICB) must inform us of:
- additions
- deletions
- changes
They must notify us by using one of the relevant proformas on our website.
The completed form should be returned to prescriptioninformation@nhsbsa.nhs.uk.
The ICB and contractor must complete the notification of bank and correspondence details proforma.
The ICB must forward the form to the Customer Payments team at contractorpayments@nhsbsa.nhs.uk.

Change of ownership
If a change of ownership occurs where a company has bought a business on a non debts and liabilities basis, we'll issue a new contractor code.
If the previous owner held a Private CD Pharmacy contractor code, we'll issue a new Private CD dispensing code to the new owner.
