How should a prescription with chargeable and exemptions items be declared on the end of month submission?

When a prescription contains chargeable and exempt items, you should add this to your end of month submission as follows:

• add the exempt item(s) to the exempt from patient charge items total 
• add the chargeable item(s) to the patient charge paid items total
• add the form to the patient charge forms total
