An item will be classed as requiring reconstitution if it's an antibacterial, antiviral or antifungal preparations listed in 5.1, 5.2 and 5.3 except 5.3.1 of the British National Formulary (BNF). They must be for oral administration and require reconstitution from granules or powder.

Payment will be made to the nearest pack size to completely cover the order when the quantity reconstituted from an original pack(s) is unavoidably greater than the quantity ordered and it's not been possible for you to use the remainder for or towards supplying against another prescription.

This will not apply to tablets listed in BNF chapter 5.1. Reimbursement will be made for the quantity ordered. See Clause 11 'Broken Bulk' to claim payment if you are unable to readily dispose of the remainder.
