If your doctor, midwife, or health visitor cannot apply online on your behalf for a maternity exemption certificate, they must submit an FW8 application form.

The FW8 must be signed by both the applicant and the doctor, midwife, or health visitor completing the form.

When sending the FW8, it must be an original document and not a photocopy or email attachment.

Primary care practitioners can order FW8 application forms and RE26 return envelopes on the Primary Care Support England (PCSE) portal.

Integrated Care Board (ICB), Sub ICB Location (SICBL) and Local Authorities can order FW8 application forms and the RE26 return envelopes from Xerox (UK) Ltd. Your organisation must be registered to use this service.
