If your maternity exemption certificate has been lost or stolen, contact us to request a replacement.
You'll receive your replacement certificate within:
- 24 hours if you have a digital certificate
- 10 working days if you have a paper certificate
If you need any medication before it arrives, tick box ‘D’ on the back of your prescription. The pharmacy will accept this and mark it as 'evidence not seen' on the prescription.
