If your maternity exemption certificate has been lost or stolen, you can view your certificate details online.
You do not need a copy of your certificate to claim free prescriptions.
If you can see your certificate details online, tick box ‘D’ on the back of your prescription. The pharmacy will accept this and mark it as 'Evidence not seen' on the prescription.
You can contact us using our online form if you cannot find your certificate details online or need a replacement certificate. You must select 'Maternity Exemption' on the form.
We’ll send your certificate details by email. You'll receive this within 24 hours.
If you need a replacement certificate sent by post, it can take up to 10 working days for this to arrive.
