If your maternity exemption certificate has been lost or stolen, contact us to request a replacement.
A replacement certificate takes 10 working days to arrive. If you need any medication before it arrives, tick box ‘D’ on the back of your prescription. The pharmacy will accept this and mark it as 'evidence not seen' on the prescription.
