If you paid for NHS treatment before finding out you were entitled, you may be able to claim a refund.
Refunds must be claimed within 3 months of the treatment completion date.

England
Download and complete an HC5(D) form.

If you receive a War Pension payment or an Armed Forces Compensation Scheme payment, you must send the form by post to the Service Personnel and Veterans Agency. The address is shown on the HC5(D) form.
You must include a copy of your receipts with the HC5(D) form.
The receipts must show the date and amount paid.
We cannot accept proof of payment from:
- credit card receipts
- bank statements
- treatment plans
It takes up to 8 weeks from the date your application is received for your refund to be paid.
Refunds for costs in England will be paid by Bacs (Bankers Automated Clearing System) transfer. You must give your bank account details on your HC5(D) form.
Scotland
Download, print, and return an HC5 refund form with your original receipt.
The HC5 form tells you where to send it.
The receipts must show the date and amount paid.
We cannot accept proof of payment from:

It takes up to 8 weeks from the date your application is received for your refund to be paid.
