If the applicant is incapable of understanding the HC1 application form, a representative can sign it on their behalf. This includes if the applicant has dementia or learning difficulties.

If this does not apply, you cannot sign the application for another person.

If the applicant can sign the form, you can fill in it in for them but they must sign the declaration.

If you're making a digital application, you do not need to physically sign the form. We'll ask you to provide your details at the start of the application and to agree to the declaration at the end.
