If you and your partner live in the same household and your partner's details were included on your original application, their name will be printed on your certificate.
If their name is missing from your certificate, contact us directly for assistance.
If your partner's details were not included on your original application, you must return your existing certificate and reapply on a new application.
If you and your partner do not live in the same household, your partner must complete an application in their own right.
