If a member is unable to physically sign their own application form, we can accept the form if the member has made a mark on it in the presence of a doctor.

The doctor must confirm the member is aware of what the mark is for.

If the member is unable to make a mark on the application form, only an individual with an enduring power of attorney (POA) signed before 1 October 2007 can sign the form on their behalf.

This also includes a POA or Court of Protection (COP) document signed on or after 1 October 2007.
