If a member is unable to physically sign their own application form, we can accept the form if the member understands what the application is for and has made a mark on it in the presence of a doctor.

The doctor must confirm the member is capable of understanding the application and what the mark is for.

If the member is mentally incapable of making an application or is unable to make a mark on the application form, only an individual with a valid power of attorney (POA) or Court of Protection (COP) can sign the form on their behalf.
