If you’re paying into the NHS Pension Scheme, you must contact your employer who’ll tell us about your name change.

If you’re no longer paying into the Scheme, you must put your name change in writing including your SD or National Insurance (NI) number.

If you're receiving your NHS Pension, we must see evidence of your name change.

We'll accept:

  • a copy of the deed poll document
  • the original or a certified copy of the marriage certificate
  • a copy of the Decree Absolute
  • your original or a certified copy of your birth certificate if you're using a previous surname
