1 Voluntary resignation or retirement
2 Employee dismissed – benefits are being withheld. You must not tell a member about this unless they mention it.
3 Redundancy
4 Sickness, absence or unpaid sick leave.
11 Employee is no longer pensionable as they’ve reached the upper age limit or reached 45 years service.
13 Assistant Practitioner becomes Principle Practitioner.
14 Member has died in service. If the member died after they’d left your employment but you’ve not sent in their termination form, you must contact us to tell us about the members death.
15 Linked employment.
16 Opted out of NHS Pension Scheme (NHSPS) but employment continues.
