Below is a table that confirms what forms should be sent to HR Shared Services (HRSS) through the Document Transfer Service (DTS).

Please note:  No transactional forms should be sent to HRSS via email:
 

Form Name When is it used?
New Employee Starter Form (NESF) Uploaded to the DTS when a new employee starts
Non Recruitment New Starter Form Uploaded to the DTS
Employee Change Form (ECF) Uploaded to the DTS when an employee's details require amending in ESR, example include but are not limited to:
- Additional payments
- Future dated changes
- Secondment amendments
- Salary
- Change of role that is not part of a recruitment
Secondment Control Form (SECF) Uploaded to the DTS when an employee is seconded in or out of the organisation
 Disclosure and Barring Service (DBS) check Information Form  Uploaded to the DTS with a Job Description (JD) and proof of identity. 
Maternity Leave Application Form Uploaded to the DTS, usually with the MAT B1 Form.
MAT B1 Form (Maternity Certificate) Uploaded to the DTS, usually with the Maternity Leave Application Form
Paternity Leave Application Form Uploaded to the DTS
Adoption Leave Application Form Uploaded to the DTS
Shared Parental Application Form Uploaded to the DTS
Flexible Hours Form Uploaded to the DTS
Flexible Retirement Form Uploaded to the DTS



Forms created by the HRSS Contracts Team Administrators:

Internal Recruitment Form (IRF) Created in Trac when an internal applicant is recruited to a role
Candidate Enrolment Form (CEF) Created in Trac when an external applicant is recruited to a role
