Please note: No transactional forms should be sent to HRSS via email:

Secondment Control Form (SECF) | Uploaded to the DTS when an employee is seconded in or out of the organisation |
Disclosure and Barring Service (DBS) check Information Form | Uploaded to the DTS with a Job Description (JD) and proof of identity. |
Maternity Leave Application Form | Uploaded to the DTS, usually with the MAT B1 Form. |
MAT B1 Form (Maternity Certificate) | Uploaded to the DTS, usually with the Maternity Leave Application Form |
