When completing an FP17O please ensure you follow the below tips on completing the form to prevent any claiming issues or rejections.

Date of Assessment same on Appliance fit CoT the same as DoA on Assess and Review CoT

If the Date of Assessment (DoA) on the initial Assess and Review claim matches the date on the active treatment, the claim will fail validation. You must either delete the initial 'Assess and Review' claim or change the Date of Assessment (DoA) on the active treatment claim, if applicable.

Patient Charge showing incorrectly on a Claim

This may be due to the patient being on a waiting list for active treatment and has turned 18 years old, before treatment can commence. In this case, you will need to contact your Sustainability and Transformation Partnership / Local Health Board to discuss the removal of the patient charge.

Submitting more than one Active Course of Treatment (CoT)

If you have submitted more than one Active Course of Treatment without a Treatment Complete claim in between this will cause the claim to reject.

Date of Referral

The 'Date of Referral' is mandatory if 'Date of Assessment' is 01 April 2019 or later.

Commissioner Approval when a Patient is 18 or over

When submitting a claim for a patient that is 18 or over at the 'Date of Referral' there is a Yes/No Box to complete.

If this has been approved by the Commissioner and 'Yes' is entered on claim, this will be translated to code 9177 on the FP17O.

IOTN and Aesthetic Component

An IOTN score is mandatory on all Abandoned/Discontinued/Treatment Complete claim forms.

Aesthetic Component became mandatory for all claims submit after 01 April 2019.
