If you permanently live in a care home and the local authority helps with the cost of your care, you can apply for help with health costs.

To be entitled to help you must have less than £23,250 (£24,000 if resident in Wales) in savings, investments, or property.

You can apply online if you either:

  • live in a care home permanently
  • have recently left authority care and have less than £6,000 in savings

If you cannot apply online, you must complete a paper HC1 (SC) form.

How you get a HC1(SC) depends on where you live.

England

You can request an HC1(SC) from Xerox by calling 0300 123 0849.

Wales

You can request an HC1(SC) from the Welsh Government publications line 0345 603 1108 (select the option for health publications).

Scotland

If you are resident in Scotland, you can collect an HC1(SC) from your local pharmacy.
