The field on the FP17 can record the DCP who’s provided treatment for the patient. Other dental staff, for example dental nurses providing chairside assistance, must not be recorded here.
A nominated performer number must be included on the FP17.
When completing an FP17 you must include:
- the type of DCP (such as a dental therapist or a dental hygienist)
- the DCP’s GDC number. This will be validated against the GDC register on Compass.
Both fields must be completed. If you only complete one of the fields the FP17 will reject.
