You must complete the recheck request form from our website to tell us of the error.

You must include the:

  • item name
  • strength
  • presentation
  • quantity

For EPS prescriptions, you must also include the: 

  • prescription UUID
  • date and time the claim was submitted

The completed form must be returned to the Customer Payments Team by email or by post to:

GP practice

You must request a recheck of your prescriptions by contacting our Customer Payments Team.

You must include:

  • your practice details
  • the month the claim was sent with
  • the items not dispensed

You can send this to us by email or by post.
