If a Local Dental Committee (LDC) needs to change the bank account that we pay the collected Levy deductions to, they must contact the Dental Services Finance team and request a Bank Mandate form.

The Bank Mandate form can be requested by emailing dentalfinance@nhsbsa.nhs.uk.

The LDC must return the form to dentalfinance@nhsbsa.nhs.uk and the Dental Services Finance team will make the change in Compass. 

These requests will be completed as soon as possible and aim for this to be completed before the next payment run.
