Completed BUR forms can only be accepted by email. They must be sent to heiforms@nhsbsa.nhs.uk.

You must complete the forms in full by entering information in all boxes in each section before sending to us. We cannot accept partially completed forms.

BUR form What form is used for Other information
Dates of Attendance Form (BUR100 MED) To confirm term dates for each course.  
Withdrawal Form (BUR101W) To confirm a student’s withdrawal from the course.  
Maternity Withdrawal Form (BUR101W MAT) To confirm a student has a period of authorised maternity, paternity, or adoption leave.  
Resumer Form (BUR101R) To confirm a student has resumed on their course following a period of withdrawal.

The form cannot be accepted if it's been issued before the date the student returned to study. You must complete the form on or after the confirmed return date on the form.

Maternity Resumer Form (BUR101R MAT) To confirm a student has resumed on their course following a period of authorised maternity, paternity, or adoption leave.

The form cannot be accepted if it's been issued before the date the student returned to study. You must complete the form on or after the confirmed return date on the form.

Section 1 of the form asks for Commissioning Authority (LETB) authorisation details - this is only needed if on returning the student will exceed the maximum period allowable for NHS Bursary funding before they're scheduled to complete the course.

Extension Request Form (BUR102 MED)

To confirm a student needs an extension to their course.

The form cannot be accepted if it was sent more than 6 weeks in advance of the extension.

If the extension is more than 24 weeks, tell the student to complete a new application.

Course Change Form (BUR102 MED) 

To confirm a student has had a change in circumstances such as moving from part-time to full-time.

 

Confirmation of Medical and Dental Student Eligibility to Apply Form (previously BUR99 MED) To confirm a student is approaching their NHS funded year.

If a student is intercalating in that year, the form must still be completed.

If the student is intercalating at a different university, it must be sent by the student's Medical or Dental university.

If the student is completing their intercalation year in a later year than their first NHS funded one and will be attending a different university, the university where they're to attend their intercalation must send a Course Change Form (BUR102 MED).

If the student will be in study for more than 210 days during their intercalation year, we’ll also need a Dates of Attendance Form (BUR100 MED) form sent by the university where they’re to attend their intercalation.
