If you're receiving Universal Credit (UC), you must also update your address with the Department for Work and Pensions (DWP).

If your name or date of birth is incorrect or you've changed your name, you must send us evidence by email to healthy.start@nhsbsa.nhs.uk or by post to:

If you're receiving UC, you must also update your name with the DWP.

We'll post a new NHS Healthy Start prepaid card to you within 10 working days of receiving your evidence.

When you receive your new card, you must destroy the old card.
