Name or date of birth

If your name or date of birth is incorrect or you've changed your name, you must send evidence by email to healthy.start@nhsbsa.nhs.uk or by post to:

If you're receiving Universal Credit (UC), you must also update your name with the Department for Work and Pensions (DWP).

We'll post a new NHS Healthy Start prepaid card to you within 10 working days of receiving your evidence. When you receive your new card, you must destroy your old card.

You must also update your address with your benefit office.

If you've moved to Scotland, your NHS Healthy Start claim will be closed and we'll reclaim any funds from your balance. You can apply for Best Start Foods.
