Submit a Change of Circumstances form.
You must provide evidence of your name change such as a:
- marriage certificate
- deed poll
If you do not have an official document proving your name change, you can provide a letter on headed paper from a professional person confirming:
- your previous name
- your new name
- your date of birth
- their name
- their address
- their telephone number or email address
If it's a sensitive subject and no outside bodies have been involved, we may accept a letter from a family member.
