If you're supported by a local authority, you must complete an HC1 (SC) form.

You can apply online if you have less than £6,000 in savings.

If you cannot apply online, you must complete a paper HC1 (SC) form. You can request this by:

calling Xerox on 0300 123 0849 if you live in England
calling the Welsh Government publications line on 0345 603 1108 if you live in Wales
going into your local pharmacy if you live in Scotland
