If you've received an enquiry letter, our checks were unable to confirm you had a valid exemption certificate:

  • at the time of your dental treatment
  • when your prescription was collected

Your details on your qualifying benefit must match the details with your dental practice or GP surgery.

If your details do not match, you can confirm them online. If you do not have access to the internet, contact our helpdesk.

If a match is found, your case will be closed.

If a match is not found, you must send evidence using our online form, or by email or post.

If your name or date of birth does not match our records, you must send evidence to confirm your correct details:

If you’re sending evidence by post, you must send a photocopy of the original document. If you send an original document, we’ll return this with your response.

When sending your evidence, you must include your: 

  • reference number
  • full name
  • date of birth
  • full address including postcode
